My Diabetes School Journal is formatted to take the guesswork out of documenting carbohydrates, insulin doses and other notes.
You will speak with your school nurse and/or teacher daily about your child’s diabetes. In addition to your verbal communication, documentation of daily discussions and treatment are important.
Each page has a dedicated space for parents and school caretakers to document the day. This includes a dedicated space for snacks, lunch, school activities as well as an area for additional notes of things that happen throughout the day. It also includes pages for weekly reflection and review.
This nine-month, five-day-week journal is not dated, so you can start using it at any time of the year, in any school system in the world.
